ACBID Violation Report Form

I understand that submitting my observations here does not constitute a direct, formal complaint to the City of Lakewood. Instead, I am notifying the Alameda Corridor Business Improvement District (ACBID). The ACBID will then use my report to facilitate solutions, and/or submit official complaints to the City of Lakewood, and/or solve the issue themselves.
